To order a report, you must have the current version of Adobe Acrobat Reader installed on your computer.  Please click on the link to obtain a free download.

Now that you have Adobe Acrobat Reader, click on the order icon to begin.  The form may be filled-in on the screen or printed and completed by hand.  "Help Notes" are provided and marked by a (?) in the left-hand margin.  Simply double-click to view the note and click the (-) in the upper left corner to close it.  When completed just Print, Sign and FAX...that's it.  Thank you for ordering from Personnel Screening Services.
